About Family Outreach Center
Family Outreach Center (FOC) is a community bridge organization that provides substance use disorder (SUD) treatment in Grand Rapids, Michigan. It is a part of the greater FOC network that offers community resources for families, children and indigenous populations as well as other behavioral health care for the region. FOC works in partnership with the Michigan Federation for Children and Families, Health West, Network 180, Urban Core Collective, United Way and countless local community organizations to provide support to residents at a grassroots level.
Recovery Management for Sustained Support
Their Recovery Management (RM) is specifically designed as a community outreach program that supports SUD management. This also includes dual diagnosis care for other mental health disorders and can include clinical care referrals, access to individual or group counseling, and supportive referrals for additional services that might help you achieve and maintain sobriety.
This is a collaborative solution that pairs an experienced, master’s level social worker with community services, recovery coaches and a case manager to help people get sober. To be eligible for this opportunity, you must be a Kent County resident with an SUD who has a documented history of unsuccessful sobriety attempts in the past along with a co-occurring mental health diagnosis.
Outpatient Support
Whether your addiction is less severe or you’re sober and want to maintain support, outpatient care is a smart start. This can include individual and group therapy as well as family therapy sessions. If you are religious, faith-based counseling is also available along with referrals to self-help groups and education classes.
